Street Medicine Peer Navigator - Field-Based Advocate
Healthcare in Action
Healthcare in Action is seeking a Peer Navigator to support members experiencing homelessness in the San Fernando Valley. This patient-facing role involves motivational coaching, housing and resource navigation, and coordinating care with the Navigation Center and HIA team. You will assist with coordinating care plans, documenting encounters in the care management system, and connecting members to local services. #J-18808-Ljbffr Healthcare in Action
