Street Medicine Peer Navigator - Field-Based Advocate
Healthcare in Action
Healthcare in Action is seeking a Peer Navigator to support members experiencing homelessness in the San Fernando Valley. This patient-facing role involves motivational coaching, housing and resource navigation, and coordinating care with the Navigation Center and HIA team.
