Description: The Registered Nurse implements professional nursing practice centered on the patient as a whole person and the creation of a caring environment focused on healing and wellness. Nursing practices are based on evidence and current research as well as hospital and department policies and procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. The RN will coordinate, ensure, and provide direct patient care activities on the unit through the provision of nursing rounds, assessments, and treatments. The RN will facilitate admission, transfer, and discharge activities while providing patient and family-centered education. Care coordination and individualized treatment planning will be conducted as applicable. The RN will perform and document a comprehensive assessment in accordance with RN scope of practice and hospital policies and procedures. Cultural, age, and developmental considerations will be taken into account during patient assessments. Patient risk factors and presenting complaints will be identified. Patient health information will be obtained from family or others as necessary. Objective and subjective data will be collected and documented. The initial admission assessment, if obtained by other members of the healthcare team, will be verified. A problem list will be developed, prioritizing patient needs based on input from the inter-professional team as appropriate. Actions and interventions relevant to the patient’s culture and age will be designed. Desired outcomes will be determined in coordination with the patient and family in accordance with the medical plan of care. Patient care therapies and procedures will be fully implemented with empowerment and clinical autonomy within the established policies and procedures. Procedures, treatments, and processes will be explained to the patient and family to ensure understanding. The RN will initiate actions to alleviate patient stress and discomfort and will recognize, respond to, and communicate changes in patient condition appropriately. Ongoing assessments and responses to procedures and therapies will be conducted and documented using the nursing process according to hospital policy and established standards. Priorities for the care of multiple patients will be set, ensuring effective utilization of time and energy relative to patient acuity and care processes. Tasks will be delegated appropriately, and team members will be overseen in the performance of activities regarding patient care. Accuracy and completeness of delegated patient care provided by other team members will be monitored in the implementation of the plan of care. Educational needs of patients related to prioritized problems will be identified and addressed in consideration of their level of comprehension. The discharge process will be facilitated by executing discharge orders in a timely manner, identifying potential discharges, and assuring the performance of discharge teaching in coordination with others as indicated. Input will be provided into patient/nurse assignments considering available staffing, unit activity, and patient acuity. Active participation in unit-based activities in support of organizational values and priorities is expected. A positive group dynamic and healing environment will be promoted through words and actions. Nursing excellence will be assured in accordance with accepted standards, evidence-based practice, and current research. Participation in department quality improvement activities and core measures, such as patient satisfaction and monitoring of documentation, is required. A holistic perspective regarding patient care will be used throughout the nursing process.
