Job Summary The Resource Navigator is responsible for providing culturally competent outreach, face- to-face case management, and care coordination services to individuals and families with complex health and social service needs. Focusing on young adults 16+, older adults, individuals 18+ with disabilities, immigrants, refugees, LGBTQIA, Veterans, individuals for whom English is not their primary language, and other vulnerable populations. The Resource Navigator connects clients to essential community resources including health care, housing, food, behavioral health, and other social services. This position plays a critical role in building trusted relationships within diverse communities, conducting client-centered assessments, developing care plans, and maintaining strong partnerships with local service providers. Resource Navigators work collaboratively across internal departments and with external agencies to ensure clients receive timely, appropriate, and effective support to improve overall well-being and self-sufficiency. - Conduct culturally and linguistically appropriate outreach to promote awareness of community-based services and programs.
- Maintain a caseload of clients with complex social and health-related needs.
- Perform comprehensive, client-led assessments and develop individualized care plans.
- Coordinate services and referrals across social services, healthcare, and behavioral health systems.
- Provide ongoing advocacy and follow-up to ensure successful navigation of services and client goal achievement.
- Facilitate education and support for clients to build self-advocacy and system navigation skills.
- Identify service gaps, observe trends in community needs, and participate in program development and quality improvement efforts.
- Collaborate with internal teams and external partners including healthcare providers, housing authorities, and community organizations.
- Organize and participate in community events, workshops, and networking activities to expand program visibility and outreach reach.
- Maintain accurate, complete, and timely documentation in accordance with program and funder requirements.
- Assist with interpretation and translation services as needed (for bilingual staff).
- Support the coordination and facilitation of regional networking events to enhance cross-agency collaboration and capacity building.
- Bachelor's Degree in Social Work, Human Services, or a related field with at least two years of relevant experience; or Bachelor's in unrelated field and four years of experience in social services; or Associate Degree or equivalent coursework with four to six years of human services experience (may require funder approval).
- Demonstrated experience working with diverse, low-income, immigrant, and refugee populations.
- Strong ability to build trust and engage individuals in culturally competent and trauma-informed ways.
- Familiarity with local social services, healthcare, and housing systems.
- Strong written and verbal communication skills; ability to prepare documentation and translate materials where required.
- Proficiency in Microsoft Office and other case management tools.
- Strong organizational skills, attention to detail, and ability to manage multiple priorities.
- Commitment to racial equity, social justice, and community empowerment.
- This is a hybrid position that requires work at a Neighborhood House site two to three days per week and community base field work.
- Must have reliable transportation to travel between Neighborhood House sites, client homes, and community events
- Bilingual skills in a language spoken by the local community are highly preferred.
- Must pass Neighborhood House background check and Washington State bi-annual re-check.
- Current First Aid Certification (within 30 days of hire, maintain current certification during employment).
- Current CPR Certification (within 30 days of hire, maintain current certification during employment).
- Sexual Harassment Training (within 30 days of hire).
- Child Abuse and Neglect Training (within 2 weeks of hire).
- Mental Health First Aid training (within 60 days of hire).
