Case Manager Mental Health Professional- New Start

Job Overview Case Manager Mental Health Professional (New Start) – Full‑time position located at Thompson Center, Aurora, IL. Responsible for case management and coordination of behavioral health treatment for clients recently released from the Illinois Department of Corrections. Schedule Monday–Friday, 9a.m.–5p.m. or 11a.m.–7p.m.; one Saturday per month. Flexible to meet client and program needs, including evening/weekend/rotating holidays and on‑call shifts. Job Responsibilities Provide face‑to‑face and telephone counseling, utilizing personal recovery experience, active listening, and motivational interviewing to help clients develop personalized recovery plans. Complete initial IM+CANS assessments and other required documentation in a timely manner. Maintain accurate case records, including scanning into Cx360 per agency procedures. Attend clinical supervision at least once per month and additional sessions as assigned. Meet monthly productivity standards by providing direct services to clients. Acquire and maintain certifications in First Aid, Adult CPR, Non‑Violent Crisis Intervention (CPI), Medication Administration, and other relevant trainings. Develop and implement crisis and treatment plans, collaborating with clients and providers, following Rule 132, payer, and accreditation standards. Participate in treatment planning meetings and complete assessment tools (LOCUS, IM+CANS, consents, privacy practices, etc.). Authorize, re‑authorize, and complete spend‑down paperwork as required. Recognize emergency situations and take appropriate action, completing incident reports and case notes. Advocate for clients’ rights and provide training to empower individuals with disabilities. Engage in community outreach to secure emergency and permanent housing, establishing relationships with hotels, shelters, and landlords. Provide services that integrate all facets of clients’ lives, including mental, emotional, and physical health; employment, education; social and family relationships; substance use; and housing needs. Provide care that adheres to the Mental Health Recovery Model and clients’ rights. Experience with court services and criminal justice preferred. Bilingual preferred. Ability to communicate in person, by telephone, and in group settings. Physical ability to stand, walk, navigate stairs, and work in clients’ homes and community settings. Physical Demands The role requires frequent standing, walking, and stair navigation; services may be provided in clients’ homes and various community settings. Equipment The position involves use of standard office equipment such as computers, phones, copiers, and filing cabinets in a professional office and off‑site community settings. Additional Requirements Flexibility to work evening/weekend/rotating holidays, on‑call shifts, and non‑scheduled shifts. Must drive a passenger vehicle; maintain a valid Illinois driver’s license and automobile liability insurance. Ability to drive yourself and clients in personal or agency vehicle; may need to work at multiple locations. Complete required training within 30 days of hire.
