The OTA I educates patients and their family members and ensures the safety of the patient. The OTA I is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The Company adopts the occupational therapy scope of practice as outlined by the state of employment.KEY RESPONSIBILITIESProvides skilled occupational therapy services in a patient’s home according to the Occupational Therapist plan of careObserves, records, and reports patient reactions, changes, and progressAssesses functional problems and provides intervention and, in conjunction with the occupational therapist works toward goals to improve patient functional statusPerforms occupational therapy procedures and diagnostic test on patients for whom they have been medically prescribedGuides the patient in his/her use of therapeutic, creative and self-care activities for the purpose of improving functioningCommunicates effectively with physician, therapist, and coworkersParticipates in case conferencing promoting coordination of careEducates staff in carrying out specific occupational therapy modalities when appropriateEducates patient and family on the appropriate treatment procedures and supportive activities and safety measuresInstructs patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devicesHelps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisionsFosters independence in patients and their families to prevent dependence on the agencyListens to patient and family membersCompletes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.Documents all aspects of treatment, assessments, and patient educationMaintains active OTA license in OhioCommunicates with scheduler any changes as neededCompletes chart audits at agency site as requestedParticipates in all on-call requirements and case conferencesAttends in-service trainings and mandatory agency meetingsParticipates in orientation/training of new employeesMaintains a professional appearance as a representative of the companyUnderstands and practices agency policies and proceduresDrives to patient’s primary location per scheduled visit.Daily attendance at assigned visit locations.Documents all aspects of subsequent visits within 24 hours of visits.Completes and submits all required documentation within specified company requirements.Follows plan of care as permitted within the scope of practice for a Licensed Occupational Therapist Assistant, per the state of employment.Other duties as assigned.QUALIFICATIONSMinimum of one year of experience as an OTA in an acute setting is required.Home care experience is desired.Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workers.Knowledgeable on therapy best practices.Ability to make appropriate occupational therapy judgments.Ability to identify a situation and handle it with the best possible solution.Detail-oriented and observant.Disciplined style of work ethic with the ability to prioritize and be timely.Valid driver’s license and auto insurance with your name as a listed driver.EDUCATION AND CREDENTIALSOccupational Therapist Assistant with a current license in the state of employment.PHYSICAL DEMANDSThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job with or without reasonable accommodations.KnowledgeReading, Speaking, Writing English - Constantly 66%-100%Communications Skills - Constantly 66%-100%Computers / PDA - Constantly 66%-100%PhysicalWalking - Constantly 66%-100%Bending - Constantly 66%-100%Standing - Constantly 66%-100%Sitting - Constantly 66%-100%Driving - Frequently 34%-66%Lifting up to 50 lbs. with or without assistance - Frequently 34%-66%Stretching/Reaching - Constantly 66%-100%Climbing - Frequently 34%-66%Hand/finger dexterity - Frequently 34%-66%Stooping (bend at waist) - Frequently 34%-66%Sensory ActivitiesDistinguish smell/temperature - Constantly 66%-100%Hearing/Seeing - Constantly 66%-100%Talking in person - Constantly 66%-100%Talking on the telephone - Constantly 66%-100%Hearing in person - Constantly 66%-100%Hearing on the telephone - Constantly 66%-100%Vision for close work - Constantly 66%-100%Other Sensory Activities N/ATRANSITIONAL WORK IS AVAILABLE (ALTHOUGH SOME JOBS MAY REQUIRE SPECIFIC TRAINING) SUCH AS: ANSWERING PHONES, FILING, COMPUTER WORK/DATA ENTRY, ATTENDING TRAINING/CONTINUING EDUCATION, REVIEWING SAFETY MANUALS & OTHER EDUCATIONAL MEDIA, AND ASSISTING OR PERFORMING OTHER TASKS WITHIN RESTRICTIONS ASSIGNED. #INDCHRI7We’ll help you put your passion for patient care to work.
