Therapist - Speech Language Pathologist

Job Summary:
A Speech Language Pathologist is responsible for evaluating, diagnosing, and treating speech, language, and communication disorders in patients. The role involves working in long-term care facilities, providing therapy, and collaborating with healthcare teams to enhance patient outcomes.
Responsibilities:
- Assess patients' speech, language, cognitive-communication, and swallowing abilities to develop individualized treatment plans.
- Implement evidence-based therapy techniques to improve communication and swallowing functions.
- Document patient progress and update treatment plans in electronic medical records accurately.
- Collaborate with medical staff, caregivers, and families to ensure continuity of care.
- Maintain current licensure and adhere to ASHA standards and regulatory guidelines.
- Educate patients and their families on strategies to support communication and swallowing in daily activities.
Qualifications:
- Hold a current NYS license as a Speech-Language Pathologist.
- Possess eligibility for or hold the Certificate of Clinical Competence in Speech Language Pathology from ASHA.
- Demonstrate excellent assessment skills and communication abilities.
- Have prior experience working in long-term care (LTC) settings.
- Proficient in MS Office, Epic, and EMR software systems.
- Knowledge of PDPM reimbursement system.
