Job Summary:

The Community-Based Outpatient Therapist provides individual and family therapy to children, adolescents, and their families through a combination of office-based, school-based, and home-based sessions. Services are designed to be flexible and responsive, meeting clients where they are, both physically and emotionally. This position requires a strong commitment to community outreach, timely documentation, clinical integrity, and flexibility in scheduling, including evenings as needed.

Duties & Responsibilities:

Responsibilities include, but are not limited to:

• Provide outpatient and community-based psychotherapy, including in-home, in-school, and in-office services.
• Deliver services to children, adolescents, and families using a strengths-based, culturally competent approach.
• Complete comprehensive psychosocial assessments of assigned clients.
• Develop and implement individualized treatment plans with measurable, attainable goals in collaboration with clients and families.
• Provide crisis intervention, case management, and referral services as needed.
• Maintain a flexible schedule to accommodate client needs, including evening appointments.
• Attend and actively participate in team meetings, clinical supervision, and peer case reviews.
• Uphold professional boundaries and treat clients with dignity and respect.
• Use sound judgment and remain calm in emergency or crisis situations.
• Apply knowledge of psychiatric diagnoses and clinical interventions according to DSM-5 criteria.
• Ensure timely and accurate documentation in accordance with agency policies and Health Information Services (HIS) requirements.
• Maintain confidentiality and adhere to all HIPAA and agency guidelines.
• Support clients in accessing community resources, attending appointments, and achieving treatment goals.
• Engage clients and families in care planning and shared decision-making.
• Promote client independence while respecting diverse values, lifestyles, and perspectives.

Additional Requirements
• Must be comfortable traveling to clients' homes and community locations; reliable transportation is required.
• Must provide a valid driver’s license and current car insurance upon hire and annually thereafter.
• Must maintain a clean driving record throughout employment.
• Maintain all professional licensure requirements necessary to practice clinical therapy in the state.
• Participate in continuing education for license maintenance and clinical development.
• Complete all required incident reports and adhere to infection control protocols.
• May occasionally serve on agency committees or assist with program development.

Requirements:

Master’s degree in Social Work, Counseling, Psychology, or a related field.
• Bilingual candidates preferred.
• Prior experience with community-based services or outpatient therapy strongly preferred.
Actively pursuing or holding independent clinical licensure (LICSW, LMHC, LMFT, etc.).
