Geriatrics-Focused Primary Care Physician - Value-Based Care

Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based care to Medicare populations at our neighborhood clinics in Brooklyn. You will join large care teams, focus on quality outcomes, and participate in a compensation model that rewards outcomes and preventive care. New graduates are encouraged to apply.
