Job Overview The Family Support Worker (FSW) is responsible for establishing and maintaining a regular and long-term relationship with families and providing in-home education and supportive services to new and expectant parents. Program Info The Healthy Families program offers home visitors who work to link parents to existing social service and health care resources, promote positive parenting, and support healthy child growth. The program typically lasts from pregnancy through the child’s third birthday and is available in English and Spanish. Key Responsibilities Outreach families referred through Central Intake or a Service Provider. Use a standardized screening tool to determine eligibility for home visitation services. Interview/Conduct the Parent Survey to identify strengths and risk factors for ongoing support. Assist families in increasing their support system and problem‑solving capacity, empowering them to advocate for themselves and their children. Help families establish goals and develop a plan to achieve those goals. Assess the normal growth and development of the target child. Identify and refer to supportive agencies, including healthcare appointments. Requirements High School Diploma or GED equivalent with at least two years of experience working with children aged 0–3 and their families. Knowledge of normal child growth/development and parent-child relationships. Experience working with community agencies and implementing referral processes. Ability to travel throughout New Jersey and attend required core and ongoing training. Acceptable driving record and use of an insured vehicle. Preferred: Infant mental health endorsement level I or II. Proficiency in the first language of the community if serving a bilingual population. Compensation & Benefits Base Pay: $21.64 per hour.
