About the Position

• Provide comprehensive physical therapy assessment and evidence-based treatment for patients with a wide range of diagnoses and complexities in the  acute inpatient setting .

• Develop, implement, and modify individualized treatment plans and goals.

• Collaborate effectively with interdisciplinary teams, including physicians, nurses, social workers, and other therapists, to coordinate patient care and discharge planning.

• Educate patients and family members on the plan of care, treatment goals, and safe mobility techniques.

• Maintain accurate and timely documentation compliant with hospital and regulatory standards.

• Contribute to the continuous improvement of acute rehabilitation services.

Requirements

• Current unencumbered Oregon Physical Therapy (PT) license required.

• Healthcare Provider Level Basic Life Support (BLS)certification required.

• Must be able to  travel to multiple points of care as required by department needs (e.g., inpatient unit, various clinic locations).
