Physical Therapist (PT) – Full-Time

$45–$50/hour

Ultimate Therapy is seeking a Physical Therapist (PT) to join our rehabilitation team. This position provides the opportunity to work with residents in a skilled nursing and post-acute care setting, helping them improve mobility, function, and overall quality of life.

Position Responsibilities:

• Conduct physical therapy evaluations and develop individualized treatment plans based on resident needs and goals
• Provide therapy services focused on mobility, gait training, balance, strengthening, endurance, transfers, and pain management
• Collaborate with PTA staff, nursing, physicians, and interdisciplinary team members to coordinate resident care
• Monitor resident progress and adjust treatment approaches as appropriate
• Educate residents and families on therapy goals, exercises, and safety strategies
• Maintain accurate and timely documentation in accordance with facility and regulatory requirements

Qualifications:

• Active Physical Therapist license required
• Strong clinical reasoning and communication skills
• Skilled nursing experience preferred but not required
