- Assist clients with tasks within the scope of a home health aide based on the Plan of Care (POC).
- Communicate effectively with administrative staff regarding client/family needs assessments, including reporting any significant changes in client condition.
- Provide care as outlined in the Home Health Aide Care Plan and accurately document the cares provided.
- Contribute to continuity of care, effective communication between all care team members, and regular participation in Case conferences/Team Meetings.
- Apply concepts of infection control and universal precaution in coordination/performing client care activities to protect both staff and patients.
- Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency. Completes training or in-services in a timely manner as assigned by your supervisor.
- Meet mandatory continuing education requirements of the Agency/licensing board.
Supervision: N/A Requirements Experience:
- (1) year experience as a home health aide or certified nursing assistant in a home health setting. Successful completion of formal training in nursing arts may be substituted for the qualifying experience, subject to applicable State law, regulation, or rules.
- Ability to establish and maintain effective working relationships with all segments of the staff, and the Agency leadership team.
- Shall be able to read, write and comprehend English.
- High School Graduate or equivalent
- Home Health Aide Certificate or other similar certification held in good standing. Certification requirements may vary by state and/or participating program.
- A formal certificate of completion for an HHA/CNA training program that meets or exceeds standards established by Health and Human Services. Completion of all in-service hours per year. Ongoing requirements may vary by state and/or participating program.
- Current CPR certification.
- Current driver's license in good standing. The employee is responsible for renewing their driver's license before it expires to continue employment.
- Reliable transportation and auto liability insurance.
- On-going employment as a HHA always requires maintaining a valid HHA certificate in the state where the caregiver practices in and a valid CPR certificate. The employee is responsible for renewing these before they expire to continue employment.
- Prolonged standing/walking is required.
- The ability to lift/transfer up to 50 lbs.
- Requires working under stressful conditions to meet deadlines and patient needs and make quick decisions and resource acquisition.
- Meet patient/family individualized psychosocial needs.
- Requires hand-eye coordination and manual dexterity.
- Works in patients' homes in various conditions.
- Possible exposure to blood-borne pathogens, bodily fluids, and infectious diseases.
- Some exposure to unpleasant weather; PRN emergency call.
- Ability to travel locally and work a flexible schedule.
