Requirements

• Required certification: Physical Therapist
• 1+ years of experience
• Key Requirements Summary: 1. Experience: 2 years required for submission 2. Cases: Outpatient PT setting 3. Certifications: BLS required; all certs must be AHA or American Red Cross and in hand at time of submittal; copies required at submission 4. License: CA License required; valid state license required at submission; Nursys within last 2 weeks 5. Education: Graduate of accredited Physical Therapy school approved by American Physical Therapy Association with B.S., B.A., M.A., M.S., or D.P.T. degree 6. EMR: Epic experience preferred 7. Shifts/Call: 5x8s, 0800-1700, no floating or on call 8. RTO: Only one holiday will be approved 9. Returning Staff: Must include former manager name during submission 10. Travel/Local: 50-mile travel requirement; locals accepted at lower rate 11. Background: Must disclose any background hits or DUIs prior to submission; 7-year work history verification required 12. Modules: 1-5 hours average, completed during orientation via e-learning; non-billable within 16 NBO hours 13. Travel Experience: Not specified as required 14. Scrubs: Not mentioned 15.
