TITLE
Registered Nurse - Home Health

SCHEDULE
8 hours per shift

ASSIGNMENT DETAILS
Contract Length: 14 weeks
Guaranteed Hours: 40 hours per week

REQUIREMENTS
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years Home health experience
- Proof of current CPR certification
- Current driver's license
- Reliable transportation and insurance

ROLES & RESPONSIBILITIES
- Assess patients in stable and emergent situations to identify their physical, psychosocial, and environmental needs.
- Develop, implement, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Provide patient and family education regarding care, diet, medications, and home safety.
- Supervise and provide clinical direction to home health aides and nursing assistants.
- Monitor assigned cases for compliance with third-party payer requirements.
- Perform on-call responsibilities as assigned and serve as a liaison among patients, families, and healthcare professionals.

COMPLIANCE / NOTES
- Must meet mandatory continuing education requirements.
- Ability to work flexible schedules and travel locally.
- Maintains confidentiality of patient information.
