Home Care Field Registered Nurse (RN) – Warren County, NY | $85KLocation: Warren County, New YorkCompany: At Home Care PartnersSalary: $85,000 per yearPosition: Full-Time Field Registered Nurse (RN)Take Your RN Career Beyond the Traditional Care SettingAt Home Care Partners is seeking a compassionate and experienced Home Care Field Registered Nurse (RN) to join our clinical team serving patients throughout Warren County, New York.This full-time position is ideal for an RN who values independence, enjoys developing meaningful relationships with patients and families, and wants to provide personalized nursing care directly within the community.As a Field RN, you'll travel to patient homes and community-based settings throughout Warren County to complete clinical assessments, coordinate Plans of Care, supervise home health aides, and help ensure each patient receives safe, consistent, and individualized care.What You'll Do• Travel to patient homes and community-based settings throughout Warren County• Conduct comprehensive in-home nursing assessments• Perform head-to-toe evaluations, including medication and diagnosis reviews• Develop, review, and update individualized Plans of Care (POC)• Complete required home health aide supervisions• Provide clinical guidance and support to aides and caregivers• Orient aides to each patient's Plan of Care and individual care needs• Monitor patients for changes in condition and communicate concerns appropriately• Collaborate with patients, families, caregivers, aides, and office staff• Complete accurate and timely clinical documentation• Help ensure continuity, safety, and quality of patient care• Maintain compliance with applicable healthcare regulations and company policiesWhat We're Looking For• Current and active New York State Registered Nurse (RN) license• Ability and willingness to travel throughout Warren County and assigned service areas• Medical Professional Liability insurance• Strong clinical assessment and critical-thinking skills• Excellent written and verbal communication abilities• Strong organizational and clinical documentation skills• Comfortable using Gmail and Microsoft Word/Excel• Ability to work independently and effectively manage a field-based schedule• Professional, reliable, compassionate, and patient-focused• Must meet all company hiring and onboarding requirementsPreferred Experience• Home care or home health nursing experience• Community health or outpatient nursing experience• Conducting in-home patient assessments• Completing aide supervisions• Developing, reviewing, or updating Plans of Care• Familiarity with Medicaid, Medicare, or MLTC plans• Bilingual Spanish/English is a plusCompensation & Benefits Salary: $85,000 per yearAt Home Care Partners offers:• Competitive compensation• Weekly direct deposit• Health insurance• Dental insurance• Vision insurance• 401(k) plan• Transit benefits and dependent care accounts• Verizon Wireless and mobile internet discounts• Company vehicle and parking access, where applicable• Supportive team culture• Opportunities for advancementBuild a Career That Makes an ImpactAt Home Care Partners is a nonprofit 501(c)(3) home care agency committed to providing compassionate, high-quality care to patients in their homes and communities.Our Field RNs are an essential part of our clinical team.
