Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
• Active Pennsylvania Physical Therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Home health experience preferred.
• Experience with HCHB and OASIS preferred.
• Minimum education requirement: Bachelor's degree.

Assignment Details

• Facility type: Home Health.
• Shift name: Days.
• Shift schedule: Monday through Friday, day shift.
• Weekly hour guarantee: 32 hours.
• On call/weekend requirement: No.
• Coverage area: Mifflin County and Juniata County; travel between patient visits reimbursed.
• Dress code: blue scrubs.
• Must hold a valid PT license.
