Leads complex patient care activities, supports clinical guidelines and policy development, and contributes to quality-improvement initiatives that enhance safety and outcomes. Functions with a high degree of autonomy and clinical judgment. Works collaboratively with prescribers and the multidisciplinary healthcare team to optimize patient outcomes and support evidence-based, cost-effective therapy. Participates in multidisciplinary committees and research initiatives. Provides education to patients, students, residents, pharmacists, and other healthcare practitioners. Minimum Education and Experience: Graduation from an accredited College of Pharmacy resulting in B.S. or Pharm D. degree Completion of a post-graduate year 1 (PGY1) ASHP accredited residency program or equivalent experience (3 years general practice) required Completion of a post-graduate year 2 (PGY2) ASHP accredited residency program or equivalent experience (3 years specialized practice) required 5+ years of experience in clinical pharmacy practice requiredCompletion of an ASHP accredited Post-Graduate Year 1 Pharmacy Residency is equivalent to 3 years of practice experience; completion of an ASHP accredited Post-Graduate Year 2 Pharmacy Residency is equivalent to an additional 3 years of practice experience (6 years of experience total for PGY1 and PGY2 training) Pharmacist licensure in applicable state. Board Certification in applicable area required within 2 years of employment. BLS, ACLS and/or PALS may be required based on practice setting.
