Social Worker

Location: Wind Crest by Erickson Senior Living Job Description Join our team as a Temporary Social Worker. In this role, you will be responsible for providing social work services to the residents and family members of Continuing Care. Compensation: $35-$36 an hour (depending on experience) How you will make an impact Completing admission assessments, MDS, and other required documentation in accordance with State and Federal regulations Educating residents/families of the importance of advance directives and facilitating completion, signature, and tracking of documents Coordinating the Social Work function from admission to discharge to include room changes and transition planning Providing psychosocial support to residents and families to ensure a successful transition to Continuing Care Participating in care/service plan meetings with family members Providing bereavement support to residents, family members, and staff What you will need Social Work degree (BSW or MSW) required, or minimum education requirements as per state licensure. Minimum of 3 years’ experience in working with the older adult population in post-acute/ long term care preferred. Knowledge of geriatric population and dementia.
