Home Care Physical Therapist

Pride Health is actively hiring a Physical Therapist (PT) to join our client's growing Home Health team. Compensation Local Pay: $65.00 - $75.00/hour (W-2) Travel Pay Package: Up to $3,000/week gross (includes weekly non-taxable stipend)* Location: Grants Pass, OR, 97527 Weekly Hour Guarantee: 32 hr Productivity Expectations (points): 30 Coverage Areas: Josephine County. Shift schedule (days, start & end times): Mon- Fri am to 4:30 pm On Call/Weekend Requirement: no Will Milage be reimbursed between visits, from first patient to last patient (Y/N)? Y Key Responsibilities Conduct comprehensive home health physical therapy services, including Start of Care (SOC), initial evaluations, routine revisits, re-evaluations, and discharge assessments , while developing individualized treatment plans, improving patients' functional mobility, and meeting a productivity expectation of 30 points . Deliver evidence-based therapy interventions, monitor and document patient progress accurately in Homecare Homebase (HCHB)/PointCare , collaborate with physicians and the interdisciplinary team, educate patients and caregivers, and ensure compliance with Medicare, agency, and home health documentation standards across the assigned coverage areas. performing Oasis assessments using HCHB, supervising PTA , Performing admissions, recerts, 30 day assessments and discharges.
