Physical Therapist - Travel Contract

Minimum Requirements Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active Wisconsin PT license or ability to obtain state licensure prior to the assignment start. Current BLS certification from the American Heart Association or American Red Cross may be required. Experience working in skilled nursing facility settings preferred. Experience with geriatric populations, restorative nursing programs, and common SNF documentation systems preferred. Ability to treat patients with mobility limitations, fall‑risk precautions, and post‑acute care needs preferred. Assignment Details Facility Type: Skilled Nursing Facility. Location: Sheboygan, WI area, covering multiple skilled nursing campuses within the region. Assignment Start: Late May 2026 (start date provided by client). Floating: Candidate may be required to float between multiple campus locations within the Sheboygan area during the assignment.
