To discuss start and end times **How you'll make an impact in this role** + Assess complex emotional and social barriers to develop personalized care plans that address each patient’s unique needs and preferences. + Coordinate with interdisciplinary teams to streamline the transition from admission to discharge, ensuring a safe and continuous journey of care. + Advocate for patient rights and preferences within the medical system to ensure the care provided is respectful and person-centered. + Empower patients and families through education on financial and community resources, helping them build the stability needed to manage their diagnosis. **What minimum requirements you'll need** **Licensure / Certification / Registration:** + Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
