Requirements: Must have acute inpatient experience; Level 1 experience required; Epic experience

Responsibilities:

• Manage inpatient cases within a hospital setting, ensuring effective patient care coordination.
• Collaborate with the healthcare team to develop and implement discharge plans.
• Document patient progress and maintain accurate records throughout the shift.
• Support hospital operations during your scheduled shifts and respond to patient needs.
• Adapt to patient care requirements as dictated by unit protocols and care plans.
