Artemis Practice Services Georgia LLC
Primary Care Physician
Old Fourth Ward (O4W) | Atlanta, Georgia
Serving one of Atlanta's most dynamic and diverse communities.
There's a Better Way to Deliver Primary Care
At Ascend Medical, we believe great primary care goes beyond diagnosing illness and prescribing treatment. It means understanding the whole person and partnering with patients to improve their long-term health.
That's why we've built an integrated model of care where physicians work alongside behavioral health providers, nutrition professionals, Advanced Practice Providers (APPs), care coordinators, and other members of an interdisciplinary team. Together, we address not only medical conditions, but also the behavioral, lifestyle, and social factors that influence health.
We're seeking a compassionate, collaborative Primary Care Physician (MD/DO) to join our Old Fourth Ward practice. This is an opportunity to practice evidence-based medicine in an environment that values collaboration, innovation, prevention, and meaningful patient relationships.
If you're looking for a practice where you can focus on delivering exceptional care while working alongside a team that shares your commitment to improving lives, we'd love to meet you.
About Ascend Medical
Ascend Medical is transforming healthcare through an integrated, patient-centered model that treats the whole person—not just individual symptoms or diagnoses.
Our physicians partner with behavioral health professionals, nutrition experts, APPs, care coordinators, and other clinical team members to provide comprehensive, coordinated care. By addressing physical health, behavioral health, nutrition, lifestyle, prevention, and social drivers of health together, we help patients achieve better outcomes and healthier lives.
Our mission is to improve the health and well-being of the communities we serve by delivering compassionate, high-quality, whole-person care through collaboration, innovation, and clinical excellence.
Why You'll Love This Opportunity
At Ascend Medical, you'll practice medicine as part of a highly collaborative team—not in isolation.
You'll have the opportunity to:
- Build meaningful, long-term relationships with your patients.
- Deliver evidence-based primary care supported by an integrated team of behavioral health providers, nutrition professionals, APPs, and care coordinators.
- Practice proactive, preventive medicine that focuses on improving long-term health rather than simply treating illness.
- Help shape and grow an innovative whole-person care model.
- Work in a culture that values collaboration, compassion, accountability, continuous improvement, and clinical excellence.
Position Summary
The Primary Care Physician provides comprehensive outpatient primary care services, including preventive care, acute care, chronic disease management, and health promotion. The physician serves as the clinical leader for their patient panel while collaborating with Ascend's interdisciplinary team to develop individualized care plans that support whole-person health.
The physician also provides clinical oversight and collaborative supervision for Advanced Practice Providers (APPs), helping ensure consistent, high-quality, patient-centered care across the practice.
Key Responsibilities
- Provide comprehensive outpatient primary care services to patients across the lifespan.
- Diagnose, treat, and manage acute and chronic medical conditions using evidence-based clinical practice.
- Perform preventive health visits, annual wellness visits, routine physical examinations, health screenings, immunizations, and age-appropriate preventive care.
- Develop individualized treatment plans that integrate medical care with behavioral health, nutrition, preventive care, and lifestyle interventions whenever appropriate.
- Identify opportunities to improve patient outcomes through Ascend's whole-person care model by leveraging behavioral health, nutrition services, care coordination, and other interdisciplinary resources.
- Collaborate closely with physicians, APPs, behavioral health providers, nutrition professionals, care coordinators, nursing staff, and other members of the care team to deliver coordinated, patient-centered care.
- Provide clinical oversight and collaborative supervision for APPs in accordance with Georgia law, organizational policies, and evidence-based practice.
- Support APPs through consultation, case review, mentorship, and clinical guidance to promote consistent, high-quality patient care.
- Coordinate care with specialists, hospitals, community providers, and other members of the patient's healthcare team to ensure seamless continuity of care.
- Engage patients through education, shared decision-making, and individualized care planning that empowers them to take an active role in their health.
- Maintain timely, accurate, and complete documentation within the electronic health record.
- Participate in quality improvement initiatives, clinical protocols, population health efforts, and best practices that support value-based care and excellent patient outcomes.
- Maintain compliance with all regulatory, clinical, quality, and organizational standards.
- Foster a welcoming, respectful, and inclusive environment for patients, families, and colleagues.
Whole-Person Care at Ascend
Whole-person care is more than a philosophy—it's how we practice medicine every day.
We recognize that lasting health outcomes require addressing not only physical health, but also behavioral health, nutrition, lifestyle, prevention, and the social factors that influence well-being.
Our physicians don't carry that responsibility alone. They work alongside an interdisciplinary team that expands what's possible for each patient, creating coordinated care plans that improve outcomes while strengthening the patient experience.
This collaborative approach allows our physicians to practice medicine more comprehensively, support healthier patients, and build stronger relationships through truly integrated care.
Who You Are
You're a physician who believes healthcare is about more than treating disease.
You enjoy building relationships with patients, collaborating with colleagues, and continuously improving the way care is delivered. You value prevention as much as treatment, embrace innovation while maintaining high clinical standards, and enjoy working in an environment where teamwork leads to better outcomes.
You believe medicine is at its best when patients are treated as whole people—not simply a collection of diagnoses.
Our Culture
At Ascend Medical, collaboration isn't just encouraged—it's fundamental to how we care for patients.
We believe exceptional outcomes are achieved when physicians, APPs, behavioral health providers, nutrition professionals, nurses, care coordinators, and support staff work together toward a common purpose.
Our culture is built on compassion, accountability, integrity, collaboration, innovation, and continuous improvement. We value clinicians who are curious, humble, patient-centered, and committed to delivering exceptional care while supporting one another as a team.
Every member of our organization is empowered to contribute ideas, pursue excellence, and help shape the future of healthcare.
Join Our Team
If you're looking for an opportunity to practice medicine in a collaborative environment where whole-person care is more than a concept—it's the standard—we invite you to apply.
