Seeking an experienced Nurse Practitioner to perform home health risk assessments on a per diem basis. This position offers flexible scheduling, allowing you to select your own shifts between Monday and Sunday, with options for weekly or bi-weekly commitment based on your availability. Shifts may be scheduled in various areas within Hudson County, including Jersey City, Bayonne, and Union City.

Key Responsibilities:

• Conduct annual health risk assessments for Medicare Advantage plan members in their homes
• Assess medical history, review current health conditions, and evaluate medication regimens
• Educate patients regarding ongoing conditions and prescribed medications
• Identify gaps in care and provide recommendations for addressing them
• Complete all documentation and assessment activities (no prescribing or treatment plan development required)

Qualifications:

• Active Nurse Practitioner license in New Jersey
• Board certification through AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited program
• Current BLS (CPR) certification
• At least six months of recent NP experience preferred

This per diem role offers competitive compensation of $80 to $90 per evaluation, along with a mileage stipend and payment for administrative time.
