Physical Therapist (PT)

We are seeking a skilled Physical Therapist to provide comprehensive physical therapy services to patients in an acute, home care, and rehabilitation setting. The ideal candidate will have a strong commitment to patient care and the ability to develop tailored treatment plans based on physician directives.

Responsibilities

• Assess, evaluate, and update patient therapeutic, rehabilitative, and functional status while contributing to the development of a comprehensive plan of care.
• Analyze home environments and assess various physical capabilities, including muscle strength, mobility, gait, and range of motion to determine rehabilitation potential.
• Guide patients and their families/caregivers in the use and care of therapeutic appliances and establish priorities for physical therapy needs.
• Report changes in patient conditions and treatment responses to the physician.
• Initiate and supervise physical therapy programs, instructing other personnel and family members regarding treatment goals.
• Train patients in the usage of prosthetic devices as necessary.
• Oversee homemaker/Home Health Aides involved in patient rehabilitation processes.
• Identify patient and caregiver needs for additional home health services and make referrals as appropriate.
• Prepare and submit clinical and progress summaries, documenting the attainment of therapy goals and patient status changes in the health record.
• Provide consultations regarding physical therapy to families and caregivers when indicated.
• Participate in peer consultation processes and ongoing staff competency assessments.
• Comply with Title 22 Medicare Conditions of Participation, Medi-Cal, OSHA, and all relevant regulations and licensure requirements.
• Perform additional duties as assigned and requested.

Qualifications

• A minimum of 2 years of experience as a Physical Therapist in acute care, home care, or rehabilitation settings.
• Current certification in Basic Life Support (BLS) is required.
• Strong oral and written communication skills paired with effective organizational abilities.
• Flexible to work Saturdays on a monthly rotation.
• Dress code permits casual attire; scrub wear is optional.

Service Area

The service area includes various locations within Yolo County, such as Woodland, Davis, Yolo, Dunnigan, Arbuckle, West Sacramento, Natomas, Knights Landing, Capay, Vacaville, Winters, and Esparto.

Mileage Reimbursement

Mileage will be reimbursed at the GSA rate following prior approval.
