Ambulatory Care Pharmacist: Population Health & Patient Care
IHA
IHA is seeking a Pharmacist to provide ambulatory clinical pharmacy services at multiple sites, including offices in Pontiac and Livonia. The role focuses on disease management, comprehensive medication review, and population health management under a collaborative practice agreement. In outpatient clinics, you will collaborate with the medical staff to design, implement and monitor pharmacotherapy, document interventions, and support quality improvement and cost-effective care across the #J-18808-Ljbffr IHA
