Behavioral Health Case Manager Salary: $56,859 per year. Distinguishing Features of the Class This position exists in the Schenectady Police Department and involves working with the at‑risk community to help connect them to providers that are better suited to fulfill their long‑term needs. The incumbent will be embedded within the Schenectady Police Department and will report to the Office of Neighborhood Engagement, acting as an extension of its Schenectady Outreach Hub Program. They will collaborate with officers on calls relating to homelessness, substance use, and mental illness, work with those at‑risk in the field, and assist in connecting individuals to services. Direct supervision is received from the Office of Neighborhood Engagement Lieutenant, the Third Platoon Patrol Lieutenant, Administrative Services Assistant Chief, and/or the Chief of Police, with leeway given for carrying out the details of the work. Does related work as provided. Typical Work Activities (Illustrative only) Meets with individuals who present to the Schenectady Police Department requesting assistance with homelessness, substance use, mental illness, or other health‑related disparities. Assists the Behavioral Health Service Navigator as necessary. Co‑responds with general patrol officers and/or outside agencies to calls for homelessness, mental illness, and substance use and assists in linking individuals to care. Manages individual client cases, ensuring their needs are met. Maintains a caseload of at‑risk individuals until they are fully engaged in essential services. Regularly meets with members of the Schenectady Outreach Hub’s management team and conducts necessary case follow‑ups. Assists in the maintenance of computer‑based tracking and reporting systems, including the Schenectady Outreach Hub Tracker, OD Mapping, etc. Performs a variety of related duties as required. Full Performance Knowledge, Skills, Abilities, and Personal Characteristics Good knowledge of community systems available to assist individuals living with housing insecurity and often co‑occurring serious mental‑health related disparities and substance use disorders; good organizational, interpersonal communication skills, and ability to establish and maintain cooperative relationships with others; ability to prepare and maintain records and reports; ability to express ideas clearly both orally and in writing; ability to understand and follow oral and written directions; good powers of observation and analysis; the ability to maintain strict confidentiality. Minimum Qualification Graduation with an associate’s degree in social work, mental health counseling, substance use counseling, psychology, human services or a related field and two (2) years of case management experience. NOTE: Your degree must have been awarded by a college or university accredited by a regional, national, or specialized agency recognized as an accrediting agency by the U.S. Department of Education/U.S. Secretary of Education.
