Open to people of every faith, **National Church Residences** has become a leading name in senior services, retirement community management and support for people in need.***We are seeking an LPN who will share in our vision to advance better living and care for seniors!*****PURPOSE**According to prescribed policies and procedures of the organization including all applicable federal, state and accreditation regulations and under the general supervision of the VP Of Operations, Primary Care, the LPN - Advanced Practice is responsible for coordinating care, providing patient education, and managing administrative tasks through remote support for nursing facilities, patients, and caregivers.**Qualified candidates for this position offer:*** An Ohio LPN Licensure in good standing* Graduate of an accredited school of nursing/practical nursing* Minimum of 2 years clinical nursing experience in long-term care, case management, outpatient settings, or similar environment.* Must have strong working computer skills with basic professional computer software (Microsoft Office suite, Windows, etc.) and the capacity for learning new software systems quickly.* Must be self-directed with excellent organizational skills and the ability to manage tasks and assignments concurrently.**ESSENTIAL FUNCTIONS*** Communicate with nursing facilities to coordinate patient care plans and address clinical inquiries.* Provide telephonic support to patients and families, answering questions, educating on care needs, and Addressing concerns.* Act as a liaison between healthcare providers, nursing facilities, and patients.* Review and update patient care plans in collaboration with nursing facilities and providers.* Monitor patient progress and follow up to ensure continuity of care.* Assist with arranging referrals, home healthcare, or specialized services as needed.* Maintain accurate and detailed patient records in the electronic health record (EHR) system.* Document all telephonic interactions, care plans, and follow-up actions per organizational policies.* Educate patients and caregivers on medication management, treatment plans, and preventative care measures.* Provide guidance on chronic condition management and self-care practices.* Assist with scheduling appointments, follow-ups, and coordinating transitions of care.* Complete pre-authorizations and communicate with insurance providers regarding coverage or care plans.* Monitor compliance with regulatory and organizational standards.* Identify and address potential barriers to care, escalating issues as necessary.* Troubleshoot concerns raised by facilities or patients and provide timely resolutions* Participates in on-call rotation.**In return, National Church Residences offers an excellent total reward package that includes:*** **Medical** (both PPO & HDHP w/HSA) – National Church Residences pays, on average, 75-80% of all costs* **Dental, Vision, Life & AD&D** Insurance and **Flexible Spending accounts*** Paid Time Off (**PTO**) and Paid **Holidays*** **Retirement Plan** including pre-tax contribution with 100% match up to 5% of your pay* **Reimbursement**for Tuition expenses* **Employee Discounts**including Tickets, Retail, etc.* **Short-Term**& **Long-Term Disability**coverage* **Accident, Hospital Indemnity & Critical Illness** Insurance* **Wellbeing Programs** including EAP, Tobacco Cessation, Weight-loss, and more\\*Programs may vary depending on Full Time, Part Time or Contingent status**Want to know more?
