Job Summary/Purpose:

The Clinical Case Manager supports the mission of Servicios de La Raza by providing client-centered, trauma-informed, and culturally responsive clinical case management services. This role exercises independent judgment in assessing clients’ individualized needs, developing comprehensive care plans, and facilitating interventions that address mental health, substance misuse, and social determinants of health. The Clinical Case Manager coordinates internal and external resources, engages in strategic partnership-building, and ensures compliance with clinical and documentation standards. By promoting wellness, reducing barriers to care, and supporting equitable access to services, this position strengthens client outcomes, informs programmatic improvements, and contributes to the agency’s broader mission of community well-being and equity.

Job Duties and Key Responsibilities:

• Assess clients’ individualized needs via a comprehensive multifaceted assessment tool addressing social determinants of health, applying professional judgment to determine priority interventions
• Develop comprehensive case management plans and assist with care coordination, exercising discretion in prioritizing resources and interventions
• Facilitate client-centered, trauma-informed interventions and harm reduction approaches, adapting methods based on client and program needs
• Provide psychoeducation on mental health and substance misuse, tailoring content to individual and group contexts
• Plan and facilitate psychoeducation groups at SDLR office and other designated offsite locations, using independent judgment to meet learning objectives
• Participate in targeted community outreach and events, making decisions that enhance program reach and engagement
• Connect clients to internal and external resources, providing guidance and follow-up to ensure continuity of care
• Complete and review all necessary client and department documentation according to SDLR standards, ensuring accuracy, compliance, and best practices
• Maintain timely and detailed clinical records via E.H.R., meeting BHA guidelines and behavioral health policies
• Meet billable services standards according to SDLR
• Maintain confidentiality of all clients’ protected health information in accordance with HIPAA guidelines
• Engage with external community-serving organizations and establish new partnerships to support program goals
• Provide input and recommendations on programmatic improvements based on client and community data
• All members of the SDLR workforce are expected to support the agency’s community outreach, civic engagement, and recruitment efforts
• Support the agency’s fundraising objectives, which may include tasks such as active fundraising, grant development, solicitation of donations, agency ambassadorship, and informational outreach, and public speaking
• Commitment to and experience with equity, cultural relevance, diversity, social justice, and disparity reduction practices with persons of color, agencies of color, and underserved communities
• Ability to establish and maintain effective working relationships with clients, co-workers, agency partners, and the public
• Other duties as assigned

Required Education, experience and Credentialing:

• Bachelor’s degree in psychology, human services, social work, or other related field
• Ability to establish and maintain effective working relationships with clients, colleagues, agency partners, and the public
• Bilingual (Spanish/English)

Working Environment and Physical Requirements

• While performing the duties of this job, the employee is regularly required to: stand, sit, talk, hear, and use hands and fingers to operate computer, telephone, and a variety of office equipment
