VNA Care: RN Case Manager (Full-Time)

Join VNA Care's mission-driven team serving patients in the Wayland, Weston & Waltham Areas.

Schedule: Monday-Friday, 40 hours from 8a-4:30p.

Salary range: $96,000 - $121,000 for a full-time schedule, will need to adjust for part-time schedules.

Requirements:

• Compassionate, enjoy working with patients and their families, organized and ability to multi-task.
• Good standing MA RN License
• 1 year of med/surg experience; 1 year of acute care experience helpful
• Willingness to travel within a territory.
