Job Requirements

• Active Physical Therapist license required
• At least 1 year of skilled SNF and/or LTC experience required
• BLS certification required
• Experience with PointClickCare or similar EMR/charting systems required
• Background in rehabilitation, acute care, cardiopulmonary, neurology, outpatient, or home health preferred
• Ability to work independently in a fast-paced rehab environment
• Travel Physical Therapist experience preferred, but not required

Responsibilities

• Evaluate, plan, and deliver skilled physical therapy services to adult and geriatric patients
• Support functional mobility, strength, balance, and safe discharge planning
• Document patient care accurately in the facility charting system
• Collaborate with the interdisciplinary healthcare team to promote optimal patient outcomes
• Maintain a patient-focused approach while managing a typical weekday caseload
• Contribute to a positive, professional rehab environment that values quality care
