RN - Hospice

We are seeking a dedicated Hospice Registered Nurse with a minimum of one year of hospice experience. This role includes providing compassionate care to terminally ill patients in their homes or nursing facilities. The RN will work collaboratively in a small team environment focused on delivering high-quality care to patients with end-stage diseases such as cancer, heart disease, lung disease, and dementia. Responsibilities Conduct 5-6 patient visits per day, ensuring comprehensive care in accordance with patient needs. Utilize EMR system EPIC for efficient charting and patient management. Coordinate with other healthcare professionals to deliver integrated care. Monitor patient conditions and adjust care plans as necessary to enhance quality of life. Required Experience / Certifications / Licensure Minimum one year of hospice nursing experience. Basic Life Support (BLS) certification from AHA.
