Must-Have:
- Candidate CVs must include month and date on work experience and education with all gaps in employment accounted for.
- Includes some Chronic Pain Management.
- Chart closure within 72 hours.
- Schedule = 18 patient encounters per day, minimum of 18 patients per provider.
- Monday to Friday, 8 AM to 5 PM; 10-hour shifts possible.
- Mix of pediatric and adult patients (considering Family Medicine only candidates).
- Decision typically made within 7-14 days.
- Active DEA needed before the start.
- Credentialing timeframe is usually 30-60 days; six weeks is common if the provider submits application and supporting documents in a timely manner.
- Required immunizations for credentialing include 2025-2026 Flu Vaccine, TB Status/X-Ray results (within 90 days before start date), Positive MMR Titer or complete immunization series, Positive Hep B Titer or complete immunization series, Positive Varicella Zoster Titer or complete immunization series, and Tdap immunization (must be within the last 10 years).
- 30-day notice required for ending all assignments.
