Long Term Care OmbudsmanPima Council on AgingPurposeThis position visits, monitors, investigates, and documents the care of residents of assigned long-term care facilities and provides consultations to residents and their families in problem resolution and long-term care concerns.Reports ToLong Term Care Ombudsman SupervisorCategoryFull-time, Non-Exempt (Band B3)CompensationAt least $44,006 annually, dependent upon relevant education, experience.Schedule, Post of Duty, & Travel37.5 hours weekly, primarily Monday-Friday, 8:30 a.m. to 5:00 p.m. Occasional evening or weekend work may be required to meet the needs of those served.This position is located remotely. Frequent travel is required to long term care facilities within Pima County.Major Duties & ResponsibilitiesVisits residents of assigned long-term care facilities on a regular basis, as often as required by contract and resident/facility needs.Receives, investigates, advocates, and assists in resolving complaints made by residents or legal guardians of long-term facilities.Provides consultation to residents, residents' families, and facilities in problem resolution and long-term care issues.Attends resident meetings and provides technical support to resident/family councils.Educates residents and their families/caregivers with long-term care issues and community resources.Assists in education about residents' rights and the role of ombudsman to long-term care facility staff.Enters all services provided into program database in a timely manner.Essential FunctionsAttends and actively collaborates with others at all scheduled meetings and trainings.Builds and maintains a cooperative working relationship with others.Utilizes technical expertise to complete all assigned tasks with an emphasis on quality and timeliness.Works independently and collaboratively with others with minimal supervision.Performs tasks at a proficient level and handles more difficult, complex, or irregular tasks.Ensures that work adheres to local, state, and federal governmental regulations, and other applicable rules and requirements.Uses virtual collaboration platforms to perform work as necessary (e.g. Zoom or Teams).QualificationsAn equivalent combination of education and experience related to the position may be considered.A Bachelor's degree in Social Work, Counseling, or a closely related field; a Master's degree is preferred.At least two (2) years of experience working with aging and disabled populations; three (3) years is preferred.At least one (1) year of experience with community services and resources; two (2) years is preferred.At least two (2) years of experience working with individuals with behavioral and cognitive concerns; three (3) years is preferred.Additional QualificationsBilingual in English and Spanish is preferredMust pass the Long Term Care Ombudsman Exam with at least a 70% within the first three (3) months of hire.Must complete sixteen (16) hours of core-curriculum training and four (4) hours of in-the-field training (in-house training provided).Must have reliable transportation.Must successfully obtain and maintain AZ Level 1 Fingerprint Clearance and Central Registry background clearance.Knowledge, Skills, and AbilitiesKnowledge of Long Term Care facility rules, regulations, and policies.Knowledge of the aging process, issues, and health problems associated with aging and disabilities.Skill in using Microsoft Office suite and virtual collaboration platforms.Ability to create, maintain, and enter information in databases with accuracy.Ability to demonstrate a commitment to the vision, mission, and values of Pima Council on Aging and its family of non-profit companies.Ability to effectively communicate with, receive information from, and convey information to others.Ability to establish clear professional boundaries with clients.Ability to identify problems, evaluate alternatives, and implement effective solutions.Ability to manage time, organize work, set priorities, meet deadlines, and follow up on work assignments with minimal supervision.Ability to professionally represent the organization and work harmoniously with diverse populations including staff, clients, and providers.Physical & Environmental FactorsWhile performing the duties of this job, the employee is regularly required to sit, walk, stand, and occasionally climb or balance, squat, stoop, twist, turn, push, pull, kneel, crouch, crawl, and reach with hands and arms.Hearing requirements include the ability to listen and respond appropriately to conversations in person, virtually, and over phone.Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception, and the ability to adjust focus.The working environment requires regular travel to long term care facilities and providing in-person services.This position works remotely and should be able to maintain a quiet, confidential space with appropriate internet connectivity to accomplish duties.This position frequently must lift and/or move up to thirty-five (35) pounds and occasionally must lift and/or move up to fifty (50) pounds.PCOA reserves the right to modify and interpret this position description.
