Physical Therapist
Position Summary Responsible for establishing, implementing and evaluating a plan of care to restore function and prevent disability following disease, injury or loss of a body part.
Education Masters Degree in Physical Therapy or Bachelor's degree in Physical Therapy with a minimum 5 years of work experience.
Licenses and Certification Illinois license in Physical Therapy BLS
Role Specific Responsibilities Performs thorough evaluation Develops appropriate Plan of Care Implements Plan of Care Documentation is accurate, complete and timely Provides supervision to assistants, rehab techs, and students
