Pay rate Hourly: $1.00 - $500.00

Shift: Monday through Friday

Under the direction of the Chief Medical Officer, this position provides comprehensive, coordinated primary and preventive healthcare services to an ethnically diverse patient population within their appropriate scope of practice.

Functions:

Upholds the Mission, Vision, and Values of Uptown Community Health Center.

Be available to work in person as scheduled and report to work on time regularly and reliably.

Maintains a safe, secure, and healthy environment by adhering to organization standards, policies, and procedures, and to legal regulations; alerting others with potential concerns.

Communicates in a professional, clear, effective, and compassionate way with patients, visitors, and coworkers. Both in person, on the telephone, and written communication.

Evaluates and treats patients within the licensed scope of practice; appropriately refers patients for specialty evaluation and treatment. Provides preventive health advice and case management and provides consultation to other staff.

Provides or arranges for comprehensive podiatric care services in a ambulatory clinic setting, including laboratory and diagnostic workup and specialty referrals.

Renders care to UCHC patients in accordance with quality and standards as established by UCHC, the standards of care prevailing in the community, applicable law, and according to state and federal grant requirements for non-discrimination regardless of status, race, sex, religion, national origin, handicap, age, or ability to pay.

Provide patients and family members with counseling and education regarding diagnosis, treatment, and the need for follow-up as appropriate during each patient visit. Verify and document that the patient understands the diagnosis, treatment, and follow-up recommendations.

Participates in multi-disciplinary, team-based care of patients.

Participates in training, reporting, and other requirements of various grant programs.

Participates in on-site and off-site outreach and continuing education programs and ensures training to maintain license and to meet the needs of the population.

Proper and accurate documentation within the electronic medical record: recording patient examination, treatment, and test results.

Maintains patient confidentiality and protects medical office operations by keeping information confidential; cautioning others regarding potential breaches.

Participates in on-call after-hours coverage.

Licensure:

Valid license to practice the profession in the applicable state.

Board certification/eligibility based on education and specialty.

Valid DEA in the applicable state

Basic Life Support (BLS) Certification

Qualifications:

Required:

DPM degree from approved program by the Council on Podiatric Medical Education (CPME)

Residency training in the area of specialty

Preferred:

Experience with Epic application and health record

Experience working with and sensitivity toward special populations, including those with limited income, limited education, and non-English speaking individuals

Bilingual English / Spanish - complete comprehension, literacy, and fluency in both languages
