Nurse Practitioner

The Nurse Practitioner provides comprehensive primary and urgent care services in a walk-in clinic setting, delivering patient-centered care for acute illnesses and injuries, chronic conditions, and preventive health needs. Essential duties and responsibilities include:

• Providing diagnostic and therapeutic services to patients including performing history and physical exams, prescribing medications, managing acute, chronic, and preventative care, making referrals for hospital and specialty services when applicable under the direction of a collaborating physician
• Reviewing and interpreting received laboratory, radiology, and consultation reports in a timely manner and providing direction to nursing staff or directly contacts the patient with results and/or treatment recommendations
• Providing quality care to patients by following the most current evidence-based guidelines, best practices, and state, local, and federal guidelines
• Providing ongoing patient education regarding current diagnosis and plan of care
• Assisting in establishing standards of care for management of patients, in collaboration with other providers and staff
• Providing accurate documentation regarding the reason of the visit, medical history, current medication list, recommended plan of care, orders for medications, labs, and/or imaging, referrals, transition of care, and follow-up
• Identifying barriers to patient care and delivery of services and assisting with formulating the plan of care and coordination of care by making referrals to internal support services or outside collaborating organizations
• Collaborating with the Chief Medical Officer (CMO), Chief Clinical Officer (CCO), other providers, and other staff to evaluate services as well as plan and develop programs
• Participating and providing input in monthly Medical Provider meetings and other committee meetings as well as participating in Quality Improvement (QI) activities, peer reviews, Pharmacy and Therapeutics, and evaluation and implementation of clinical programs
• Reviewing current literature to remain current with advances in therapeutic interventions
• Maintaining patient confidentiality at all times in accordance with HIPAA regulations
• Other duties as assigned.

Minimum requirements include:

• Graduated from an accredited Nurse Practitioner program
• Licensed in the State of Missouri
• Basic Life Support (BLS) Certification
• Active Missouri DEA and BNDD licensure

Preferred requirements include:

• Two years' experience as a Nurse Practitioner preferably in an Federally Qualified Health Center, or a walk-in/urgent care clinic.
• Bilingual in Spanish and English
• Experience with eClinical Works (eCW)

Working conditions and demands include:

General working conditions are in a healthcare facility. As such, there is a potential exposure to virus, disease, and infection from patients while performing the duties of this job. May experience traumatic situations including psychiatric traumatized and deceased patients. Travel may be expected to community meetings, client homes, or other agencies.
