Occupational Therapist (OT) - Part-Time

Location: Taylorville, IL
Facility: Taylorville Care Center
Setting: Skilled Nursing Facility (SNF)

Continuum Therapy Partners is seeking a compassionate and dedicated Occupational Therapist (OT) to join our rehabilitation team at Taylorville Care Center in Taylorville, Illinois.

This part-time Occupational Therapist opportunity offers the chance to provide exceptional occupational therapy services in a supportive, collaborative skilled nursing facility where your clinical expertise makes a meaningful impact every day.

As an Occupational Therapist (OT/OTR) , you will work with adult and geriatric patients to improve independence, safety, mobility, and quality of life through individualized occupational therapy interventions.

• Evaluate patients and develop individualized occupational therapy treatment plans.
• Provide skilled occupational therapy services in a skilled nursing facility setting.
• Address activities of daily living (ADLs), functional mobility, coordination, strength, cognition, and safety awareness.
• Help patients improve independence and participate in meaningful daily activities.
• Monitor patient progress and adjust treatment plans based on patient needs.
• Collaborate with physicians, nursing staff, caregivers, and interdisciplinary rehabilitation team members.
• Complete accurate and timely documentation according to company, state, and federal guidelines.
• Educate patients and caregivers on therapeutic techniques, adaptive equipment, and safety strategies.
• Provide compassionate, evidence-based occupational therapy services.

• Graduate of an accredited Occupational Therapy program.
• Current Illinois Occupational Therapist (OT/OTR) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
• Strong communication, organization, and teamwork skills.
• Passion for providing high-quality, patient-centered care.
