Job Summary

Responsibilities
Provide direct patient care as directed by licensed nursing staff, including assistance with daily living activities, vital signs, and medication administration
Maintain a safe and clean environment for patients, ensuring that all necessary equipment is cleaned and disinfected
Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
Participate in patient care conferences and contributes to the development and implementation of individualized care plans
Assist with restorative care programs, including exercise, diet, and behavioral modifications
Maintain accurate and up-to-date records of patient care and services provided
