Job Overview:
We are seeking an experienced RN Case Manager for a travel assignment in an acute care hospital setting. This role is ideal for a nurse with strong care coordination skills who can effectively manage patient care plans, ensure appropriate resource utilization, and support safe and timely discharges.

 

Key Responsibilities:

• Coordinate and manage patient care throughout the hospital stay
• Perform utilization review and discharge planning
• Collaborate with physicians, nurses, and interdisciplinary teams
• Ensure appropriate level of care using InterQual criteria
• Work with patients and families to develop safe discharge plans
• Maintain accurate documentation in EMR systems
• Facilitate communication between healthcare providers and external resources

Required Qualifications:

Active RN license
Minimum 2 years of acute care hospital Case Management experience
Strong knowledge of InterQual criteria
Experience with Meditech and Midas systems
