LPN/LVN Skilled Nursing Facility (SNF) Appleton, WI

We are seeking a dedicated LPN/LVN for a travel assignment in a Skilled Nursing Facility (SNF) setting in Appleton, WI. In this role, you will deliver high-quality, compassionate patient care across short-term rehabilitation and long-term care units, collaborating with an interdisciplinary team to support patient recovery and daily health needs.

Key Responsibilities

• Provide direct patient care, including wound care, stoma care, wound vac management, tube feedings (Peg, J-tube, G-tube), and catheter care.
• Administer medications and monitor patients receiving IV fluids and antibiotics (for IV-certified candidates).
• Perform IV starts and lab draws as required by unit needs.
• Support patient care in various units, including Short-Term Stay, Rehab, SNF, and Long-Term Care (LTC).
• Maintain accurate, up-to-date patient records utilizing electronic medical record (EMR) systems.

Qualifications & Requirements

• Experience: Minimum of 2 years of nursing experience in LTC, SNF, LTAC, or Rehab settings.
• Certifications: Active BLS/CPR certification required. IV certification is preferred.
• Licensure: Valid, active nursing license with no infractions or actions.
• Flexibility: Willingness to float across facility units (Rehab, SNF, LTAC, LTC, Short Term Stay) as needed.
• Location Eligibility: Position is open to candidates residing outside a 50-minute radius of the location.

• Assignment Length: 13 Weeks
• Shift/Schedule: 5x8s Days (6:00 AM 2:30 PM)
• Estimated Weekly Compensation: $1,199.11 $1,410.71 per week
