Job Overview: Responsible for assessing, coordinating, monitoring, and providing nursing care services and treatment for The Gardens of Arkansas LLC. people. Coordinates quality improvement development, support and data collection for analysis as prescribed by the physician and Mental Health Professional.

Essential Duties and Responsibilities

Operations

· Based on the Nurse Practice Act, perform focused health assessments on personal care and support services, identify changes in status and data collections such as vital signs and medical history as required to include safety and functional assessments.

· Assist in support planning by contributing to the development and revision of support plans by providing data and suggesting potential interventions to the RN or physician.

· Functions as a member of the Support Team. Report people status changes gathered data and other observations to the RN or physician.

· Monitors people medications and treatments prescribed by the mental and healthcare providers. Provides medication and treatment education to people and caregivers as necessary.

· Assists people in maintaining optimal health through education and counseling with people, caregivers and family facing chronic conditions.

People Support

· Treat people with dignity and respect in all interactions. Remember that the staff are professionals, and we are in their homes.

· Provide supervision per person’s Individual Service Plan (ISP)

· NEVER leave a site/ program without staff coverage.

· Ensure that medication is being ordered and logged in and administered as ordered by the physician.

· Assist people with budgeting purchases, ensuring that receipts are obtained for all transactions and that accounts are balanced.

Health & Safety

Health & Safety

· Ensure the facility is always kept clean and tidy.

· Report any maintenance concerns directly to the Program Administrator.

· Ensure hygiene of each person receiving services and follow through with all scheduled medical and psychiatric appointments.

· Respond to a person’s behavioral and medical crisis immediately and according to policy.

· Proactively manage training/ corrective action for medication administration/ documentation.

Qualifications

· License/Certification Arkansas License (Required), and 1 year experience working with people with serious mental health disorders or developmental disabilities.

· Must have a valid driver’s license for all positions that require transporting people.

· Exceptional people service skills/interpersonal skills with all staff/employees.

· Must have the ability to clearly communicate with people in English both verbal and written.

· Ability to pass DHS background check, drug screen and maintain background clearance.
