Home Health Speech-Language Pathologist (SLP) | Travel Contract

Location: Mount Angel, OR

Duration: 13 Weeks

Shift: Days | Wednesday – Friday

Hours: 24 Hours Per Week (3x8-Hour Shifts)

Responsibilities:

• Provide speech, language, swallowing, and cognitive therapy to adult and geriatric patients in their homes.

• Manage a home health caseload and complete scheduled patient visits.

• Perform comprehensive clinical assessments and develop individualized care plans.

• Complete OASIS documentation accurately and efficiently.

• Document patient progress, outcomes, and care coordination activities in EMR systems.

• Collaborate with interdisciplinary healthcare teams to support patient care goals.

• Educate patients and caregivers regarding treatment plans and therapy goals.

• Manage daily travel and scheduling between patient visits.

• Maintain compliance with home health regulations and agency standards.

Preferred:

• Prior experience with Epic EMR software.

Requirements:

• Minimum 1 year of recent skilled Home Health experience.

• Active Oregon SLP License (pending Oregon license accepted).

• Current BCLS (AHA) certification.

• Direct OASIS documentation experience required.

• Valid driver's license and clean driving record.

• Reliable personal transportation required.

• Strong communication, documentation, and independent clinical decision-making skills.
