Occupational Therapist (OT) - PRN / Full-Time

Pleasant Acres Rehabilitation & Nursing Center is seeking a compassionate and motivated Occupational Therapist (OT) to join our dynamic rehabilitation team. We are currently hiring for both Full-Time and PRN opportunities . This position offers the opportunity to work collaboratively with therapists, nursing staff, physicians, residents, and families to deliver high-quality rehabilitative care that promotes independence, functional improvement, safety, and enhanced quality of life.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans based on clinical findings, functional abilities, and resident goals.

• Provide skilled occupational therapy interventions focused on improving independence with activities of daily living, mobility, coordination, strength, and overall functional performance.

• Implement evidence-based treatment strategies to support residents in achieving their highest level of independence.

• Monitor resident progress and adjust treatment plans as needed to support positive outcomes.

• Accurately document evaluations, treatments, progress notes, and other required clinical information in a timely manner.

• Collaborate with interdisciplinary team members to support resident care goals and discharge planning.

• Educate residents, families, and caregivers regarding treatment plans, safety techniques, adaptive strategies, and functional improvement goals.

• Maintain compliance with facility policies, professional standards, state regulations, and federal guidelines.

• Participate in care conferences and team meetings as appropriate.

• Ensure a safe and therapeutic treatment environment for all residents.

Qualifications

• Graduate of an accredited Occupational Therapy program required.

• Current Occupational Therapist license in the State of Pennsylvania in good standing.

• CPR certification preferred.

• Previous experience in skilled nursing, long-term care, rehabilitation, or post-acute settings preferred.

• Experience with Net Health documentation software preferred.

• Experience with PointClickCare (PCC) preferred.

• Strong clinical, communication, organization, and interpersonal skills.

• Ability to work effectively as part of an interdisciplinary team.
