Responsibilities Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes Complete comprehensive in-home assessments and deliver skilled PT visits Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention Set measurable goals, track objective progress, and adjust treatment quickly when patients stall Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving Manage treatment and outcomes directly, including PTA supervision where appropriate Coach patients and caregivers with practical home programs that actually get done Document promptly and thoroughly in Adaptive's proprietary AI-native application Requirements Active Texas PT license (in good standing) Strong clinical judgment and comfort working independently in the field Comfortable escalating changes in condition to RN/MD and documenting rationale clearly Strong communication and documentation Reliable follow-through on orders, frequencies, and patient goals Current driver’s license and comfort with daily travel across assigned territory Able to carry/transport basic therapy equipment as needed Competency in core home health PT scope Safety-first mobility work: transfers, gait training, balance/fall prevention Patient/caregiver coaching with practical home programs Hard Skills In-Home Assessments Gait Training Balance/Fall Prevention Functional Mobility Strengthening Treatment Plan Development Objective Progress Tracking Therapy Equipment Transport Soft Skills Strong Communication Reliable Follow-Through Independent Work Certifications & Qualifications Current Driver’s License #J-18808-Ljbffr
