Nurse Practitioner

Position Summary The Nurse Practitioner is responsible for both clinical care delivery and operational leadership within the Neonatal Growth & Development (GraD) Clinic. This is a dual‑role position with approximately 40–50% dedicated to direct patient care and 50–60% dedicated to clinic operations, strategy, and program development. The GraD Clinic serves infants and young children (ages 0–3) with a history of Neonatal Intensive Care Unit (NICU) admission, providing longitudinal developmental follow‑up and care coordination. The incumbent co‑leads a multidisciplinary team of 8 providers and staff (neonatologists, DBPed, psychologist, OT and clinic coordinator) and partners closely with the Medical Director to ensure high‑quality, standardized patient care while driving operational efficiency, program growth, and cross‑functional collaboration. This is a unique opportunity to combine clinical care with operational leadership in a mission‑driven, high‑impact setting. Responsibilities Clinical Operations & Administration Lead day‑to‑day clinic operations, including patient flow, staffing, and resource allocation. Design provider schedules and workflows to optimize access, efficiency, and care coordination. Develop and maintain clinic policies, workflows, and standard operating procedures. Serve as operational lead across departments within Columbia University Irving Medical Center and NewYork‑Presbyterian Hospital. Monitor performance using Epic reporting and implement improvements in access, throughput, and no‑show rates. Partner with finance teams to improve revenue cycle performance and reimbursement. Serve as a resource for Epic workflow optimization and staff training. Lead team meetings, annual retreat planning, and execution of key operational and strategic initiatives. Drive quality improvement efforts and use data to inform decision‑making. Act as primary problem‑solver for operational challenges and lead cross‑functional initiatives. Supervise and support clinic coordinator; provide coverage as needed. Clinical Care & Care Coordination Provide direct patient care (average 3–4 clinic sessions per week) for children ages 0–3 with a history of NICU admission. Conduct developmental assessments and deliver longitudinal follow‑up care for high‑risk infants and young children. Coordinate care with inpatient NICU teams to support referrals, eligibility determination, and discharge planning. Engage families prior to discharge to ensure smooth transition to outpatient care. Standardize care delivery across providers to ensure consistency and quality. Collaborate with external NICU follow‑up programs and community partners, including Reach Out and Read, to support family resources and outcomes. Perform related duties & responsibilities as assigned/requested. Minimum Qualifications Requires Master’s degree or equivalent in education and experience, plus two years of related experience. Preferred Qualifications 2-3+ years of experience as a Nurse Practitioner. NICU experience (e.g., RN background) and/or developmental pediatrics experience. Primary Care experience.
