Qualifications Graduate of an accredited practical nurse or vocational nursing program. Minimum of 1 year of nursing experience. Current and valid LPN license in the state of Illinois. Current CPR certification. Strong clinical and critical thinking skills. Excellent communication and interpersonal skills. Ability to work independently and manage time effectively. Reliable transportation and a valid driver’s license. Responsibilities Provide direct patient care as defined in the state Nurse Practice Act. Implement the plan of care initiated by the registered nurse (RN). Cover the territory of Chicago, IL , within a 30-minute drive radius. Provide accurate and timely documentation consistent with the plan of care. Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care. Participate in the coordination of home health services, appropriately reporting identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the RN and/or Clinical Supervisor. Use and/or prepare equipment and supplies effectively and efficiently, adhering to aseptic technique. Participate in personal and professional growth and development. Perform other duties as assigned by the RN. Provide healthcare instructions to the patient as appropriate per assessment and plan of care.
