Responsibilities Provide outpatient rehabilitation services in an in‑home setting for older adults. Evaluate and treat patients in the most functional environment, usually in the patient’s home. Organize and conduct occupational therapy programs to facilitate rehabilitation. Direct patient participation in selected tasks to restore, reinforce, and enhance performance. Facilitate the learning of skills and functions essential for adaptation and productivity. Maintain a flexible schedule and work in various settings. Supervise occupational therapy assistants, temporary licensees, students, and support personnel as applicable. Adhere to department and patient treatment schedules and complete timely clinical and billing documentation. Attend and contribute to patient care, staffing conferences and related meetings. Make referrals to other disciplines and agencies to address comprehensive patient needs. Consult with the rehabilitation team, including community physicians, to select appropriate therapeutic regimens. Adapt program and/or plan of care according to individual patient needs. Promote a safe environment and eliminate fire and safety hazards. Order supplies and equipment as necessary. Clean, maintain, and report repair needs for equipment. Prioritize patient service; ensure patients and families receive high‑quality care in a compassionate atmosphere. Screen, examine, and evaluate patients, including history, systems review, and application of appropriate tests and measures; integrate information and interpret findings to establish diagnosis, identify impairments, predict improvement and timeline, identify precautions/contraindications, and design plan of care. Develop and implement treatment goals and methods in collaboration with patient and caregivers; complete all related documentation and record keeping. Provide therapeutic techniques in areas such as cognitive skills, muscle strength, coordination, endurance, mobility, perceptual abilities, sensory awareness, sitting and standing tolerance, balance, activities of daily living, joint protection, work simplification, orientation, physical agent modalities, low vision, falls risk management, medication management, health and wellness, community integration and reintegration, case management skills in the home and community space. Design, fabricate, fit, and/or apply adaptive devices; train patients in their use, including orthotics and prosthetics. Select appropriate activities suited to the individual’s physical and mental capacity and interest. Instruct patients in the skills and techniques required for functional activities and evaluate their progress, attitudes, and behavior. Perform routine reexamination as needed to modify or progress the treatment plan. Perform other related duties as required. Qualifications Bachelor’s degree in an occupational therapy curriculum accredited by ACOTE standards. Initial registration from the National Board for Certification in Occupational Therapy. Valid and unencumbered occupational therapy license as required in the state of practice. Valid driver’s license or reliable, independent transportation to and from the patient’s residence.
