This role is an essential part of the CHS Community Transitions Division, working closely with the Opioid Treatment Program (OTP). In this impactful position, you will screen patients for the OTP, provide education on medications that treat Opioid Use Disorder (OUD), facilitate connections to community clinics for ongoing support after release, and submit referrals to ensure patients receive the care they need. Ensure SMI and others at risk due to medical, mental health, or substance use problems will receive a warm handoff at release from jail to their assigned provider or Mercy Care Arizona providerDevelops and coordinates community re-entry plans that address risk factors associated with recidivism to the criminal justice systemCoordinates with a multidisciplinary team regarding patient care to develop patient discharge plans, including community entitiesAssist qualifying patients in AHCCCS enrollmentObtain treatment records and coordinate care with family, community providers, and others relevant to the patient’s care and community re-entry planMaintain and update a list of community resources to support patients’ transition back to the communityMaintains HIPAA patient confidentiality in the management of informationComply with the county's policies and procedures related to the safekeeping of Protected Health InformationAdheres to all applicable professional standards and requirements, including modeling the code of ethics associated with licensure and certification, and meeting regulatory, licensure, accreditation, health assessment, and CPR requirementsWorking ConditionsTravel or floating to various clinical locations is required within a 5-mile radius.Must be able to respond to emergenciesMust maintain CPR certification and get on the floor to perform CPR/chest compressions, push medical equipment, climb stairs, and lift patients on a gurneMust be able to work inside a jail and stand/walk/bend for extended periods of time throughout the shiftMust be able to pass a fingerprint background check and maintain MCSO jail access.This position requires accurately handling a high volume of work, communicating with a culturally diverse patient population, and adapting to frequent changes, interruptions, and unscheduled taskPosition includes day or night shift work, which may include weekends and holidaysSelection ProcedureConsideration will only be given to candidates who submit online applicationsCandidates will be contacted primarily through email and their Workday online application profileMust pass a pre-employment background and/or fingerprint investigation as required by statute or policy, including drug and alcohol testing requirements for positions designated as safety-sensitiveMaricopa County is an equal opportunity employer and is committed to providing equal employment opportunities to all applicants.
