POSITION TITLE: Clinician
PROGRAM/LOCATION: San Luis Obispo FSP ICM/ San Luis Obispo
POSITION SUMMARY:
The Full Service Partnership Intensive Case Management (FSP-ICM) provides intensive, community-based services to unserved and underserved adults age 18 and older with serious mental illness and/or co-occurring substance use disorders who are experiencing homelessness or are at risk of homelessness, incarceration, or institutionalization. Under the Behavioral Health Services Act (BHSA), FSP-ICM serves as the least intensive level of care within the FSP continuum and may function as a step-down from Assertive Community Treatment (ACT).
Using a trauma-informed, person-centered approach, the multidisciplinary FSP-ICM team provides outreach, engagement, assessment, individualized service planning, therapy, intensive case management, integrated behavioral health treatment, medication support, housing assistance, vocational services, rehabilitation, crisis intervention, life skills development, transportation, and linkage to community resources. Services are primarily field-based, with telephonic and video services available when appropriate.The team supports individuals in achieving stability, improving health and wellness, reducing hospitalization and incarceration, and successfully integrating into the community through ongoing support and access to after-hour coverage, which includes a twenty-four (24)/(7) hotline service and resource coordination.
HOURS/ SALARY:
Full-time, non-exempt position. 40 hours per week. Anticipated hiring range is $28.50-$36.50. Includes a generous individual benefits package that includes health, dental, vision, life, 403b retirement (with up to a 5% employer match) paid vacation/holiday time, paid sick time and a variety of helpful employee perks as well. MISSION STATEMENT:
Transitions Mental Health Association is a nonprofit organization dedicated to eliminating stigma and promoting recovery and wellness for people with mental illness through work, housing, community, and family support services.
RESPONSIBILITIES AND DUTIES:
1.Provide a wellness and recovery approach to service delivery to all clients. Foster and maintain a collaborative team approach with the FSP ICM team members. Model a client-centered approach to service delivery, bringing a “whatever it takes” approach to meeting the needs of consumers in all aspects of their treatment.
2.Function as a core member of the multidisciplinary FSP-ICM team, operating within a shared caseload model.
3.Collaborate with case managers, nurses, peer mentors, medication providers, and other team members.
4.Ensure clinical information is communicated appropriately across the team while maintaining confidentiality
5.Provide services in the community, including clients' homes, shelters, community settings, and other locations identified as appropriate.
6.Develop and update individualized, person-centered treatment plans in collaboration with clients and the FSP-ICM team.
7.Support clients in developing coping skills, emotional regulation, communication, and problem-solving skills.
8.Support clients in understanding their treatment and making informed decisions regarding their care.
9.Provide services in the community, including clients' homes, shelters, community settings, and other locations identified as appropriate.
10.Engage clients who may have difficulty participating in traditional office-based services.
11.Provide outreach and engagement to clients who are difficult to engage or at risk of disengaging from services.
12.Assess and respond to changes in mental health, including increased symptoms, suicidal ideation, or other safety concerns.
13.Coordinate with substance use treatment providers when appropriate.
14.Medi-Cal billing provides a critical portion of funding for this program. You are expected to reach documentation and billing goals as set by Team Leader and Director.
15.All direct service staff are expected to spend 50% of their work time on direct service and support of clients, unless otherwise assigned by your supervisor.
16.Maintain timely and accurate clinical documentation in compliance with County, program, HIPAA, and regulatory requirements.
17.Promote, encourage, and maintain trauma-informed practices in all services and activities. Practice and promote, both individually and in team meetings, TMHA’s Collaborative Communication Guidelines to facilitate clear, empathetic, and person-centered communication.
18.Collaborate closely with psychiatry, nursing, substance use specialists, vocational specialists, peer specialists, and case managers.
19.Provide psychotherapeutic counseling, educational, and rehabilitation services for individuals, couples, or members of a family to assist them in achieving productive social adjustments, reducing the impact of disabling disorders, and facilitating the achievement of family, vocational, recreational, and interpersonal goals.
20.Coordinate and provide linkage to behavioral health, primary care, substance use, housing, employment, and other community resources.
21.Provide crisis counseling in the field, in the office for individuals experiencing acute episodes; evaluate clients and arrange for voluntary or involuntary admission to facilities for observation and treatment. Become trained and designated to conduct evaluations and write applications for 5150 holds (training provided).
22.Provide transportation to clients as determined by the daily schedule and the needs of the client.
23.Cooperate and work with other agencies and practitioners.
24.Participate in ongoing quality assurance and program evaluations.
25.Assist with interviewing, hiring, and training new staff.
26.All other duties as assigned by the SLO Team Leader and Directors.
JOB ENVIRONMENT:
1.Daily and frequent documentation of client services on the computer.
2.Services are delivered to clients during workweek; schedules will be developed in collaboration with all teammates and management staff to ensure maximum support for clients.
3.Moderate, physical activity and frequent driving that will include the transportation of TMHA clients; may include moving or lifting up to 50 lbs.
4.Involves moderate risks that require safety precautions and the risk of exposure to potentially intense situations.
Requirements:Minimum Requirements:
1.Possession of a valid license as a Clinical Social Worker (LCSW) or Marriage and Family Therapist (LMFT); or a Registered Intern progressing toward licensure in one of these disciplines.
2.Working knowledge of: assessment, diagnosis, prognosis, counseling, and psychotherapeutic treatment of marriage, family, and child relationship dysfunctions; methods of developing and implementing a treatment plan; human service delivery system; current trends in the field of mental health; and laws and regulations applicable to mental health.
3.Ability to: perform effective counseling and psychotherapy with individuals, families, and groups; develop and maintain cooperative relationships with clients’ families, members of other professional disciplines, social service agencies, and co-workers; relate professionally with persons with serious mental disorders; identify and utilize community resources; prepare and present clear and concise oral and written reports.
4.Must have experience working with persons with severe and persistent mental illness.
5.Must have experience working with persons who are chronically homeless, homeless, or at risk of becoming homeless.
6.Must participate in the on call rotation schedule to provide services to clients after hours and on weekends.
7.Must have a strong commitment to the right and ability of each person with a severe and persistent mental illness to live in a safe living environment, develop a meaningful role in the community, and have access to competent and continuous supports and services.
8.Must have a strong commitment in clients’ rights to self-govern and create their own choices and goals.
9.Strong interpersonal skills, collaborative work style and strong oral/written and communication skills.
10.Well defined organizational and time management skills.
11.Ability to work independently and flexibly
12.Ability to handle intense situations, display good judgment and decision-making skills, and remain calm and clearheaded under pressure.

22.Conditions of employment include:
•Must be at least 18 years of age; possess a valid CA Driver’s License, at least 2 years driving experience, reliable transportation, proof of auto insurance, and DMV driving record that meets TMHA’s current vehicle insurance requirements. May be required to utilize personal vehicle and auto insurance.
•Must comply with a TB assessment (possibly a TB test if needed to confirm a negative tuberculosis result) and receipt of a criminal record clearance through fingerprinting.
•Must be able to successfully complete First Aid and CPR training.
